Birth Certificate of each of the contracting parties. Some civil registrars honor only Certified True Copy issued by the National Statistics Office (NSO). 3. Certificate of Attendance in a pre-marital counseling and family planning seminar conducted by the Division of Maternal and Child Health at the Municipal/City Hall in the same municipality or city where the contracting parties applied for the marriage license.
4. Marriage Contract. This applies if the couple has already civilly married. It should be marked ''Certified True Copy'' and issued by the city/town Civil Registry Office where the wedding has been officiated.
